COLORADO'S 5 STAR CERTIFICATION PROGRAM encourages businesses to implement safety measures beyond what is already required by public health orders and guidelines. It allows participants to operate at the next-lower level of restrictions on COLORADO'S COVID-19 DIAL. The program is completely voluntary and optional. HOW TO APPLY (Applications are now closed)

STEP 1: Scroll down and find your business sector including your sector guidebook, application, and cleaning forms. Check the current LA PLATA COUNTY DIAL STATUS.

STEP 2: Read your sector GUIDEBOOK first to understand the required public health protocols plus best practices for each and every requirement.

STEP 3: Gather all documents, statements, and narratives needed to be able to complete the application. See your sector GUIDEBOOK for details.

STEP 4: Apply online by clicking the application link for your sector when ready. A completed CLEANING SCHEDULE FORM will be required during the application process.

NEXT STEPS

Upon receipt of your application, two separate teams will review it to ensure all required information has been submitted. It is possible they will ask you to submit additional information. When your application is passed by the review team, it will then go to an inspector. The inspector will contact you to schedule an on-site inspection to verify everything in your application. At the inspection, your tables should be set with the appropriate spacing, and all forms and procedures need to be available for the inspector to review. A manager or owner should be with the inspector, and the inspection should take about an hour to complete.

If your business passes its on-site inspection, the inspector will provide the official 5-Star Certified Business sticker for placement on your door. You can then immediately open at the current capacity limit.

If at any point your application does not pass, it will be assigned to a problem solving team to help you find creative ways to address any concerns.

All certified businesses will receive unannounced follow up inspections, the first of which will occur within three to four weeks of opening. Follow up inspections are designed to ensure all protocols are in place and are being followed 100% of the time. Also, a complaint portal has been established by CDPHE. Customers who observe non-compliance with the requirements can submit a complaint. If a complaint is verified, the business will be contacted to address the situation. Repeated non-compliance with the requirements will result in a warning letter, which offers one last opportunity to correct the problem. If the problem is still not corrected, the businesses 5 Star Certification will be revoked.
